    The purpose of this position is to provide case management and care coordination to people who are participating in a Social Security funded research project.

    • Coordinate and provide access to resources that may be available at the agency, within the community, and through the study’s financial support to research participants. Some of these resources include- mental health care, short-term supported education/training, housing assistance, financial assistance, substance abuse counseling and treatment, legal assistance, etc.
    • Work directly with people served to solve problems, reach recovery goals and access or better understand any state or county benefits they may be receiving.
    • Work with a group of providers to ensure that people’s needs are understood and addressed.
    • Utilize Person Centered Thinking, Motivational Interviewing and Problem Solving Therapy to engage and work with people

    Qualifications & Benefits


    • Bachelor's degree in social work or equivalent with an emphasis in vocational rehabilitation, social services, psychology or the equivalent combination of education and experience
    • Knowledge of the IPS vocational model of service delivery and job development and job retention preferred
    • Previous experience doing case management with adults with a mental health diagnosis highly preferred
    • Knowledge of community resources within Hennepin County and how to navigate them
    • Ability to work with diverse groups including team members, people served, community partners and internal management
    • Excellent time management and multi-tasking skills with strong ability to problem solve
    • Proficient with Microsoft Office software (Outlook, Word, etc.) and interest to learn and become proficient in case management software system (Care Director)
    • Current, valid driver's license and driving record that demonstrates safe, legal driving habits
