    Responsibilities

    Support individuals in community based employment by providing follow up support services:

    • Schedule regular check in meetings with employers and individuals to enhance job retention and stabilization. 
    • Provide individualized on the job coaching and training as needed.
    • Provides an open channel of communication with the employer and team members in regards to progress and performance.
    • Ability to adapt to various work environments involving a variety of tasks, duties and employer/ employee relationships.
    • Provide case management services as needed

    Qualifications & Benefits

     Qualifications:

    • Positive and dependable person with good time management skills
    • Flexible with schedule
    • Minimum 1-2 years work experience
    • A valid driver’s license and compliance with Rule 11 and MVR background check required.
    • Access to reliable transportation
