  • Overview

    The purpose of this position is to lead a team of direct support professionals in providing excellent service for people with disabilities. The program provides services for approximately 50 individuals with severe developmental disabilities and physical challenges.

    Responsibilities

    Program Management

    • Identify program development goals and implements plans to achieve them.
    • Ensure compliance and quality within assigned programs and services and maintain working knowledge of licensing, regulatory and quality standards (ex. Licensing, CARF, 245D, MVR) and applicable funding requirements for each program.
    • Oversees and implements program evaluation/outcome measurement system for assigned areas. Ensures all required reports (both internal and external) including data management and outcome performance are completed in a timely manner.
    • Network with area businesses to identify and secure new community sites and job opportunities
    • Build and maintain effective working relationships with site contacts and maintain business partner's expectations for productivity, quality, etc.
    • Serve as a liaison with other appropriate agencies and organizations to promote the interests of Rise
    • Monitor revenue and expenses of program in partnership with the Program Director. Complete annual budget projections within timelines.

    Leadership

    • Manage multiple Service Team Leaders who oversee numerous Direct Support Professionals and job sites
    • Build and maintain a positive, effective work environment and manage team performance through direction, feedback, training and professional development
    • Lead others in focusing on providing excellent service for people served, their families, and all other people who support them; etc.
    • Ensure team members provide case management services including the efficient, timely and accurate use of documentation in our case management software system.

    Qualifications & Benefits

    Qualifications:

    • 3 years of supervisory level experience in a program providing direct support services is required. 
    • A degree in a human service related field and experience providing direct service to individuals with significant disabilities, preferably in a DTH setting.
    • Previous experience practicing person-centered practices with people with disabilities or other barriers to employment preferred
    • Must meet the qualifications required of MN 245D statute's Designated Coordinator role 
    • Strong self-management, thinking and interpersonal skills
    • Ability to direct and motivate others, perseverance and compassion for team members and the people we serve
    • Proficient with Microsoft Office software (Outlook, Word, etc.) and interest to learn and become proficient in case management software system (Care Director) 
    • Current, valid driver's license and driving record that demonstrates safe, legal driving habits
